We at OSF Health Care strive to care for the whole person, in their physical, emotional, spiritual, and relational dimensions. Our concern extends to all persons we encounter, turning no one away for reason of race, color, religion, or economic status. “With the greatest care and love” was an expression used by our Foundress, Mother M. Frances Krasse, in her final letter to the congregation in 1885. This is the “golden thread” in the OSF (Order of St. Francis) story, linking us with the founding spirit, current service, and vision for the future.
In achieving outstanding performance improvement in the Home Health Care category, we have focused on performance improvement in several areas. Visit scheduling and adjusting has been one of our primary focuses. Patient visit schedules are plotted and managed by computer, thus making it easier to forecast visits. If a patient requests a schedule change or staff change, this too is updated in the computer schedule by clerical staff and/or the supervisor.
Reacting to patient and family concerns to scheduling inconveniences and adjustments has played a critical role in our success. Contact with patients and family members from both field and office staff, regarding schedule adjustments, has allowed us to receive a rating in the 99th percentile when compared to AHA Region 5 facilities.
Our service area is divided into three “Care Areas” in which field staff is assigned a primary care area. This has allowed the number of different staff members entering a patient’s home to be kept at a minimum, and has greatly improved our ability to maintain continuity of care.
